When can members get the vaccine?
The County of San Diego is currently vaccinating individuals 12 and older. Individuals who are 12 to 17 years old are eligible to receive the Pfizer vaccine. Individuals 18 and older are authorized to receive any of the CDC-recommended vaccines. You do not need to contact your doctor before your appointment unless you have a specific medical question.
How can members get vaccinated?
How can members who are homebound get the vaccine?
Sharp Health Plan and our plan medical groups are doing outreach to members who have been identified as homebound (or unable to leave their home due to illness or old age) to ensure that anyone who can't get to a COVID-19 vaccination site is still able to get vaccinated. If a member is homebound and their primary care physician or plan medical group has not reached out to them to schedule their COVID-19 vaccination, please have the member contact our Customer Care team so that we can assist them.
How can members replace a lost vaccination card?
Members can now show digital proof of vaccination. Members who were vaccinated in California can go to myvaccinerecord.cdph.ca.gov to access it. (We recommend the latest version of Chrome, Firefox, or Safari for the best experience.)
Members may also request proof of vaccination through the San Diego Immunization Registry (SDIR) by filling out the SDIR form. For more information, members can contact SDIR at 1-619-692-5656 or firstname.lastname@example.org.
Is the vaccine a covered benefit for members?
Yes. The vaccine is covered at $0 under members’ preventive care benefits.
