How to become a contracted broker partner.

Sharp Health Plan is different from other health plans. We are the only locally based nonprofit commercial health plan offering plans designed by San Diegans, for San Diegans. Before beginning the contracting process, please note that we will be requesting the following information and documents in the application:

Agency contracting information:
- Tax ID
- Agency name
- Commission address
Principal agent's contact information:
- Name
- Email address
- Phone number
Copy of your California insurance license
Copy of your Errors & Omissions insurance certificate
Completed W-9 form
Covered California certification (if you are a certified agent through Covered California)

To begin the process, follow the steps below.

Email our IFP Sales team to submit your request to become a contracted broker.

Select the link to easily send our team a prewritten email.

Complete the broker partnership application.

Our team will send you a link to fill out the online application. Before you submit your information, please note:

This broker partnership contract is only for IFP and group.
Your contracted tax ID number through Covered California^™ and through Sharp Health Plan must match to avoid commission discrepancies.
Your contracting information should match IRS records to avoid discrepancies.
Please provide the most updated information and contracting documents.
The broker partnership contract will have an effective date on the first of the month after the broker partnership form is received.

Complete the required agreements.

After submitting the application, you will receive a broker agreement, business associate agreement and an Addendum via Docusign within 3-5 days. Please review and sign/initial where applicable.

After receiving the signed broker agreements, we will review your application.

After review, you can expect to receive an executed agreement within 15 days.
