Member ID is your Sharp Health Plan member ID number, and identifies you as the main contact on the account.
Statement date is the date the bill is generated. Activity posted after the statement date will be included on your next bill.
Bill period is the time frame your payment covers.
Previous amount due is the remaining unpaid balance due from a prior billing statement.
If you receive the premium assistance benefit, the premium assistance amount is the Advance Premium Tax Credit (APTC) that has been applied to your account and is automatically deducted from your monthly premium.
Amount due is the total that must be paid in full by your due date or your coverage may be canceled.
Please pay by is the due date by which your payment must be received by Sharp Health Plan.
