You may have received, or will soon receive, an IRS 1095 tax form in the mail. This form verifies you had the minimum essential health coverage during 2017. Under the Affordable Care Act, you are required to verify your health care coverage when you file your federal tax return and this form fulfills that requirement. Although you are not required to submit your 1095 form to the IRS when you file your taxes, we encourage you to keep it as part of your tax records.

What you'll receive
If you were enrolled in Sharp Health Plan during 2017 through Covered California, the health benefits exchange, then you will receive this form from Covered California.
If you were enrolled in Sharp Health Plan through your employer or you purchased an individual/family policy directly from Sharp Health Plan (not through Covered California) during 2017, then you will receive this form by January 31, 2018 from Sharp Health Plan.
If you were offered health coverage by a large employer during 2017, then you will receive this form from your employer. A large employer is generally one with 101 or more employees.
