Sharp Health Plan has issued new member ID cards for all Sharp Health Plan members, with an effective date of January 1, 2018. If you and/or your dependent(s) have not yet received your new member ID cards, submit this form to request replacement cards. Please allow 7 - 10 business days to receive your new cards from the date of your request.
